I think the most embarrassing thing was going on a job interview and having the interviewer pull up an article about a client who was considering to terminate their contract.

Pay below industry and location average, no clearly defined Career progression path, immature product and project management, poor product/release planning, quality of product suffers due to scope/schedule mismanagement, nobody ever fired for poor quality work or management, management allowed to bully others even with multiple HR complaints from different employees, mandatory "improvement" classes such as 7 Habits, etc
