We are able to assess and treat the physical, mental, social and environmental needs of the patient for less than they would typically be charged for a hospital or outpatient visit.</p> <p><strong>About the Role</strong></p> <p>We are looking for a Family Nurse Practitioner to serve as our market lead for our innovative practice model that focuses on delivering whole-person care via an on-demand network of Responders supported, via telehealth, by an interdisciplinary team. Our care model requires every member of the care team to be comfortable addressing acute or chronic physical health, mental health, and the social determinants of health impacting patient outcomes.
